I request the Management of M/s Special Rescue Training Academy Pvt. Ltd. to permit me/my family  members/guests to participate in the Swim with Lifesaver  activity and I agree that availing of such activities will be entirely at MY own risk. 

I hereby declare that I’m major of age AND, I and my family members/guests  accompanying me are NOT suffering from any physical ailments, which prohibits  me/us from availing the use of this activity/facilities.  

I further declare that I am assuming full responsibility for the safety of the minor  accompanying me and that M/s Special Rescue Training Academy Pvt. Ltd. Management or any of its  staff members or operators or any one else connected with this activity is NOT liable or NOT responsible in any respect whatsoever AND that  we/our heir and legal representatives shall NOT be entitled to claim or recover  from the Special Rescue Training Academy Pvt. Ltd. Management/their staff members in the event of  any mishap causing injury to me/my family/guests OR damage to my/our personal belongings. 

I further agree that in case of any damage caused to any equipment part of / used in the activity, while in my custody, I am liable to pay for the damages.
